New clients can request an appointment by following the steps below.

How to make an appointment.

1. Request an appointment by clicking on the APPOINTMENT REQUEST FORM.

2. HYM will send you an Intake Form to complete online.

3. Ensure you send through the relevant documentation with the Intake Form (eg. referral letter/NDIS Plan with goals) to info@healthyyoungminds.com.au.

4. Once received, you will be contacted via email to arrange an appointment.
